Our patients receive compassionate, quality care in the convenience of your home or assisted living facility. The nurse practitioner provides complete physical exams, prescriptions, immunizations, referrals for specialty providers and management of both acute and chronic illnesses.
DEMENTIA CARE
Customized and Comprehensive
The Medicare covered service of detection, diagnosis, and coordination of care for patients with dementia and their caregivers is our specialty. This home visit includes a cognition, function, and safety assessment as well as evaluation and careplanning for neuropsychiatric behaviors, review of medications, and creating a suitable plan for patients with cognitive impairment that can be done in collaboration with your current primary care provider.
PALLIATIVE CARE

This is a wonderful resource for anyone living with a serious illness such as kidney disease, heart failure, COPD, cancer, dementia and many others. The palliative care team will help patients identify and create a plan of care that can improve quality of life, help with symptoms, allow patients to age in place, and assist patients to understand their medical choices for treatment and coordinate with other health care members to provide medical, emotional and practical support at any stage of illness.
