The Care You Deserve

Since 2007 Desta Anthony, MS, ANP-C, CDP as been providing house calls for home-bound patients, providing primary care, urgent care and well-care visits. She has served various populations from private homes, structured settings, to at-risk populations in the community.  Nationally certified as an Adult Health Nurse Practitioner and a Certified Dementia Practitioner, she has worked independently as well as part of a team for Loretto PACE, currently serving as their in-house medical provider for the dementia facility at The Heritage. She has completed the CPAC Palliative Care Designation and collaborates with Hospice and in-patient Palliative care providers.
