Advanced Geriatrics is a mobile medical practice dedicated to delivering compassionate, expert care for serious illness management, cognitive care, and outpatient palliative care directly to older adults. We specialize in supporting older adults and their families wherever they call home. Our mission is to empower aging with dignity, comfort, and personalized medical attention every step of the way.
Desta Anthony, MS, ANP-C, ACHPN, CDP
Desta Anthony is a board-certified Nurse Practitioner with more than 20 years experience specializing in Geriatric care, Dementia care, and complex medical management for frail elders. She earned her Master of Science degree from Upstate Medical University and her post-master's certificate in Geriatric Neuropsychiatry from Saint Mary’s College. She is nationally credentialed in Adult Health and holds an Advanced Certified Hospice and Palliative Nurse credential. She has worked as a geriatric Nurse Practitioner at Loretto with PACE and serves as the Dementia Consultant for The Heritage and the Borer Memory Care Unit. She is involved in research as a Sub-investigator for dementia research trials with ICHOR Research. She maintains active memberships in the American Academy of Housecall Medicine, Hospice and Palliative Care Nurses Association, Gerontological Society of America, and the Nurse Practitioner Association of New York.

Christopher Norman, MSN, APRN, APHN-BC, GNP-BC
Christopher is a Board-Certified Geriatric Nurse Practitioner and Holistic Nurse. In practice since 2004 – first working as a Nurse’s Aide, then Registered Nurse, then Nurse Practitioner – Chris has been working with older adults. He holds degrees from SUNY Geneseo (BA), Dominican College of Blauvelt (BSN), Yale University (MSN), and New York University (Post-Master’s Certificate in Geriatrics) and is currently pursuing a PhD in Nursing through Binghamton University. His nursing professional interests are in aging well, wound care, holistic therapies, the living experience of the older adult, and helping people better understand dementia. Chris has presented in more than 50 public health engagements throughout Central NY, nationally, and internationally to help educate people on aging and dismantling ageism. A father, husband, avid reader, and jogger, Chris is also a certified yoga instructor and enjoys Yoga and Reiki for self-care and spiritual development.
Toni Heer, MS, CNS, RN
With over 31 years of nursing experience, including 26 years at Upstate Hospital, Toni specializes in adult complex care and transitional care nursing. She holds a master's from Walden University, completed clinical nurse specialist training at SUNY Upstate, and is certified in transitional care nursing. Toni has led initiatives to improve patient outcomes and support those affected by acute and chronic illness.
Her interests and certifications include case management, care coordination, transitions management, functional nutrition, exercise, and patient advocacy
Toni, a lifelong Syracuse resident, owns and operates a 1,000-acre crop farm in South Onondaga with her husband. They enjoy outdoor activities and care for many pets.
